SJV Awards Program Project Application Cover Sheet

Each proposal to the SJV Awards Program must be accompanied by a completed cover sheet. Just follow these easy steps:

1. Click to access the Cover Sheet form in fillable PDF or Word format.
2. Enter information in the appropriate spaces.
3. Save your work to your computer (just click the "save" icon in the toolbar of your web browser; you can also select "File" then "Save").
4. Change the name of the file to reflect your project, organization, or your own name so it can be easily identified when it is received by SJV staff (e.g., JDuberstein_CoverSheet.pdf).
5. Click "Save".
6. Email this file to the SJV along with the rest of your application materials.
